In our previous two article posts on All on Four dental implants, we took a look at the key advantages of this revolutionary teeth replacement protocol over traditional removable dentures or false teeth. These included greater bite functionality, patient comfort, smile aesthetics (both short and long term) and the preservation of good jaw bone health. In this, the final installment of a three-part series on the benefits of All on Four dental implants, we shall take a look at the cost differences between this breakthrough technology and older, more traditional method for teeth replacement.

All on Four Dental Implants: Smart Long-term Investment

In order to fully appreciate what a smart investment All on Four dental implants are, one needs to take a step back and evaluate the greater picture. All on Four dental implants provide patients with a long term teeth replacement solution that is fixed, non-removable, entirely comfortable and immediately functional. All on Four dental implants enable patients to eat all the foods they love without worrying or feeling self-conscious and they never need to be removed for cleaning. In fact, this revolutionary teeth replacement technology doesn’t come with any of the challenges and repeated costs associated with removable dentures.

This is where one needs to appreciate the difference between the initial price tag for All on Four dental implants and dentures. While the former may represent a greater investment, it is one that will benefit patients for decades (up to 30 years if cared for properly) without the need for future restorations and expenses. All on Four dental implants, in other words, constitute a one-time investment with a one-time solution to rampant tooth loss and edentulism (not having any of your original adult teeth left)!

All on Four Dental Implants versus Removable Dentures

Removable dentures, unlike All on Four dental implants, go hand-in-hand with several repeat expenses. These include:

Adhesives: Dental adhesives are required to prevent removable dentures from slipping and shifting around.

Anesthetic Gels: Because dentures rely entirely upon the gums for support, they can cause pain, irritation, inflammation and the development of sores that aren’t given a chance to heal. Anesthetic gels are necessary to make dentures more tolerable for their wearers.

Cleaning Products and Storage: Dentures harbor food and bacteria. They require cleaning after meals and at nighttime, which requires a constant investment in specialized denture products. Hygienic containers for nighttime storage are also a necessity.

Refitting: The greatest repeat expense incurred by removable dentures – one avoided entirely by All on Four dental implants – is the need for refitting as a result of bone loss in the jaw. The atrophy of bone tissue is a natural consequence of tooth loss and while All on Four dental implants help to prevent the deterioration of bone volume in the jaw, removable dentures do not. As this hard tissue wastes away, over time, dentures that once fit snugly, begin to slip more and more, until they are actually intolerable to wear. This necessitates expensive refitting every few years or so.

All on Four Dental Implants: A Final Note

All on Four dental implants really do offer edentulous and near-edentulous patients a superior solution to all their oral problems. Dentures are associated with numerous challenges and repeated expenses that can quite markedly affect one’s quality of life and self-confidence. With All on Four dental implants being such a sophisticated teeth replacement solution, patients should think very hard before wasting time and money and resigning themselves to the pain, discomfort, social embarrassment and inconvenience associated with removable dentures.

This article explains the differences between the cost of All on 4 dental implants and traditional tooth replacement techniques and how the former constitutes a far more worthwhile investment.

One of the primary concerns patients express when investigating dental implants as a solution to single, multiple and total tooth loss (a state referred to as edentulism), is cost. This especially applies to patients seeking a solution to rampant tooth loss and edentulism. Since a single dental implant can cost in the region of $4,000 it is no wonder that patients balk at the thought of replacing all of the teeth they have lost as a result of periodontal (gum) disease. Thankfully, All on Four dental implants offer all the benefits of a sophisticated and non-removable tooth replacement solution, but at a price tag that is as much as $45,000 lower than traditional dental implant protocols! With standard cases affording savings of around $25,000, there really is no reason to compromise on your quality of life by having archaic tooth replacement technology such as removable dentures and bridges fitted.

All on Four Dental Implants: Worth the Investment

The money you spend on undergoing the All on Four protocol constitutes an investment in your long term oral health and hygiene. Removable dentures come hand-in-hand with a whole host of challenges and expenses that can prove to be extremely socially and financially crippling. In the long term, denture wearers face the inevitable need for dental implants as a result of the loss of bone volume in the jaw; a consequence of tooth loss gone untreated. At some stage no amount of refitting can prevent dentures from slipping and causing acute discomfort. So, if you are edentulous or facing an immediate future of edentulism, then why not make this important investment in your long term oral health and quality of life?

In these situations, some perspective always helps: if you were in the market for a car, you wouldn’t expect to pay a paltry amount for a high performance vehicle; one that looks, works and feels superior to any other model you’d ever test driven. The same applies to dental technology. While the All on Four constitutes a greater initial investment than other more conventional techniques, such as removable dentures and bridges, it also affords massive savings over traditional dental implant protocols. And the benefits and advantages of the All on Four of course go without parallel! Now more than ever before, thanks to the All on Four technique, dental implants are becoming a solution that is accessible to people of all ages and walks of life.

All on Four Dental Implants: Key Benefits

The key benefits afforded by All on Four dental implants include:

A brand new set of non-removable teeth, almost always with a single surgery, in a single day.

Instant smile aesthetics and comfort.

Immediate functionality.

No more messy adhesives, anesthetic gels or cleaning products for removable dentures.

No more self-consciousness when you eat or speak; the All on Four is permanently affixed in the mouth and so it cannot shift around or fall out.

No need for expensive refitting every few years or so.

The All on Four is a comprehensive and long term solution to rampant tooth loss and edentulism.

This article explains how it’s possible for patients to go from not having a single original tooth left to having a full set of permanent, immediately functional and aesthetic teeth in as little as a single day.

You could be excused for thinking that the possibility of receiving a new set of permanent teeth in a single day is the stuff of science fiction. You can just imagine Tom Cruise (pre-couch-jumping incident on Oprah) clad in a space suit receiving a new flashy set of pearly whites from some complex robotic arm after a particularly rowdy brawl caused one or more of his teeth to go flying into the vacuum of space. But the technology has arrived and now, the possibility of new teeth in one day has gone from being the fantasy of the edentulous denture-wearer to the reality of tens of thousands. And this is all thanks to the placement protocol and products behind All on Four dental implants!

What Are All on Four Dental Implants?

All on Four dental implants offer edentulous (not having a single original adult tooth left) and near-edentulous patients a permanent tooth replacement solution. By placing only four dental implants in the upper and/or lower jaw, skilled and experienced dentists can affix a customized non-removable prosthetic dental bridge that looks, feels and functions like a full set of natural teeth. Unlike removable dentures, All on Four dental implants do not need to be taken out after meals and at nighttime for cleaning; they cannot shift around or fall out and they cause no damage or discomfort to the surrounding soft tissue. Most importantly, however, All on Four dental implants help to promote the health and vitality of the underlying jaw bone tissue, which would otherwise atrophy and waste away. This loss of bone volume in the jaw is responsible for that aged and sunken appearance we typically associate with the elderly and denture-wearers.

All on Four Dental Implants: Making New Teeth in a Day Possible

Ironically, it is the loss of bone volume that inhibits patients from receiving dental implants using traditional placement protocol. Frequently, bone grafting surgery is required in order to augment and encourage the growth of new and healthy bone tissue before dental implants can be considered a viable tooth replacement option. But with All on Four dental implants, the need for lengthy, painful and expensive bone grafting surgery can almost always be avoided! So, through the ingenious design behind this breakthrough placement protocol, skilled and experienced dentists are able to place the All on Four dental implants and the customized prosthetic dental bridge in a single surgery. This eliminates the months (frequently up to a year and a half) required for the jaw to heal before implants can be placed as well as the expenses and inconveniences associated with multiple consultations, surgeries and time off from work.

A Final Note on All on Four Dental Implants

Not only do All on Four dental implants offer an immediately functional, aesthetic and comfortable solution to multiple and total tooth loss, but they also facilitate a road to oral rehabilitation that takes a single surgery – not multiple, as is the case with traditional dental implant protocol – and a single day, rather than months and even years.

This article provides a brief overview of the life, education and success of Dr. Paulo Malo as the innovator of the ‘All-on-Four’ dental implant protocol.

All on Four Dental Implants: An Introduction

Dr. Paulo Malo is a world famous implantologist whose name resonates powerfully throughout the fields of dental implantology and fixed oral rehabilitation; and the reason for his international acclaim? He is the pioneer of ‘All on Four dental implants’; the implant protocol that has largely revolutionized the way modern dentists approach the treatment of tooth loss and edentulism (not having any original adult teeth left). Since his days of clinical work and medical research, Dr. Paulo Malo has branched out immensely and is now a leading educator in the field of implantology and the revolutionary ‘All on Four’ technique.

All on Four Dental Implants: The Beginning

Dr. Paulo Malo was born in 1961 in Angola, South Central Africa. He embarked upon his tertiary education at the prestigious University of Cape Town, which lies at the very southernmost tip of the continent. His science degree enabled him to explore subjects from nuclear physics to marine biology, but it was a chance meeting with Dr. Christian Barnard – the first doctor to ever perform a successful heart transplant – that really altered his perspective. From that moment on, Dr. Paulo Malo knew he wanted to make an important contribution and his field of choice was medicine… in dental implantology, as he would later discover.

All on Four Dental Implants and Dr. Paulo Malo’s Education in Dental Science

Political turmoil in South Africa at the time of Dr. Paulo Malo’s tertiary education prompted his parents to send him to Portugal, where he was to complete his degree. Once in Lisbon, he made the decision to study at Lisbon University’s Faculty of Dental Medicine, from where he graduated in 1989. Four years later, this extraordinary man – together with a clinical research team and one of the world’s largest implant manufacturing companies, Nobel Biocare – innovated All on Four dental implants; a revolutionary technique for tooth replacement and one that was to change the lives of tens of thousands of people around the world!

All on Four Dental Implants: The Importance of the Discovery

The innovation of All-on-Four dental implants in 1993 was recognized as one of the most important advancements to have been made in dental implantology in recent decades. But this amount of success was, for Dr. Paulo Malo, only the beginning of a long and extremely successful career. Since the early 1990’s, Dr. Paulo Malo has co-authored numerous text books and scientific journal papers, while frequently being invited as a guest speaker to prestigious universities all over the world. This is not even to mention his dedication to training other dentists to master the placement of All on Four dental implants. Dr. Brueggen, founder of The Brueggen Dental Implant Center in Houston, Texas, www.nodentures.com, was one of the first to be mentored by Dr. Malo. It is clear from this man’s involvement in all aspects of his business that his interests are as complex and numerous as his strengths and it is this extraordinary gift that has ensured the incredible international success of Dr. Paulo Malo!

This article discusses the benefits of the All on Four dental implant protocol as well as the oral conditions it is able to treat.

To those unfamiliar with All on Four dental implants, Houston teeth in an hour may seem like an unfathomable concept. Sure, you’ve heard of instant coffee, instant noodles and instant oatmeal… but instant teeth? Yes, that’s right; dentists can now replace single or multiple teeth in the lower or upper arches in just one hour. And they are increasingly able to do this thanks to All on Four dental implants. This tooth replacement protocol uses just four implants to secure a customized, non-removable bridge in the mouth. This immediately functional and aesthetic solution is ideal for those that have either already lost all of their teeth [a state referred to as edentulism], or are facing an imminent future of edentulism.

The Benefits of Houston Teeth in an Hour:

Choosing Houston teeth in an hour means saying goodbye to removable dentures forever and all the associated embarrassment and discomfort. With Houston teeth in an hour you will no longer need to hide your smile, repeat yourself due to speech impediment, or avoid your favorite foods. All on Four dental implants provide immediate relief along with pride in your smile, the restoration of eating function and in most patients, a general renewed self-confidence that may have been dampened by years of denture-wearing or unpleasant-looking natural teeth.

Thanks to incredible advancements in medical technology and tooth replacement protocol, in most cases, minimal incisions and sutures are needed for the placement of All on Four dental implants. As a result, post-operative pain, discomfort and recovery time are greatly reduced and patients can have the entire procedure completed in a single one-hour appointment. In fact, many patients return to work with a new set of teeth the very next day and are tucking into their favorite meals with confidence a few days later. Your sense of taste will be fully restored along with your self-assurance.

Houston Teeth in an Hour Explained:

Almost three decades ago, the concept of All on Four dental implants was developed by European implantologist Paulo Malo as a permanent solution for patients with multiple missing or damaged teeth. Remarkably, this alternative to traditional methods of dental implants allows patients to replace all of their teeth with the use of just four implants in a single session.

Conditions Houston Teeth in an Hour Can Treat:

Houston teeth in an hour using All on Four dental implants is an optimal solution for patients who are suffering from the following:

• Rampant tooth loss as a result of gum disease

• Edentulism [not having any original adult teeth left]

• Traditional tooth replacement techniques, such as dental bridges or removable full or partial dentures

• Failing dental implants previously inserted using traditional techniques

Houston Teeth in an Hour – A Final Note:

Houston teeth in an hour – thanks to revolutionary All on Four dental implants – are bringing the possibility of a fresh start to the tens of thousands of people in the United States that are missing multiple teeth. The results speak very much for themselves: Houston teeth in an hour are aesthetic, functional and promote both your short and long term oral health [www.nodentures.com].

This morning, at the Brueggen Dental Implant Center, we did a lower All-on-4 dental implants case for a 60 yr old gentleman from Colorado.

He was in town on personal business and learned about us from a T.V. ad. His medical history was non-contributory. Dentally, he was wearing an upper and lower denture. There was a mobile and failing unilateral subperiosteal implant on the lower left, and one had previously been removed on the lower right. Subperiosteal implants are placed beneath the “gums” and rest on, but not in, the bone.

Subperiosteal implants are used when there is inadequate bone volume to place a dental implant into the bone, and the treating doctor is not aware or trained in other options. They experience a very high failure rate. I have removed a number of subperiosteal implants but have never placed one.

These implants were placed in Colorado about 15 years ago when the upper and lower dentures were made. It was the doctor’s hope to attach the lower denture to the subperiosteal implants, but unfortunately, that was never possible.

This patient was displeased with the fit of his false teeth. Even with thick denture adhesives, he was unable to eat, and the lower was unstable even in speech. He was a very active gentleman and very frustrated with his situation because his previous dentist had told him that he did not have enough bone for dental implants.

We took a cat scan (a very sophisticated x-ray designed to allow precise measurements of bone in all dimensions) and determined that he did have enough bone for dental implants if the All-on-4 dental implants (a full set of upper or lower teeth fixed to 4 dental implants) technique was used.

Consultation revealed that financially doing this on the upper and lower was not feasible, so we elected to do a new denture on the upper and the All-on-4 dental implant procedure on the lower. Our in-house lab pre-made the upper denture, so when he arrived this morning, we were able to deliver that before I began the surgery.

Using conscious oral sedation, I began the surgery at 9:30 A.M. The failing subperiostel implant was removed, 4 endosseous dental implants were placed, and the surgery was completed at 10:55 A.M.

Impressions were taken for the lab and at 4:30 P.M., the patient was ready to leave with his new upper denture and new lower non-removable teeth attached to 4 implants.

His healing and recovery should be uneventful, and I anticipate making his final set of teeth in 6 months.

My last two blogs were about Mr. A, the gentleman that we did an upper All-on-Four on about 2 weeks ago and who is waiting for hyperbaric oxygen treatment, so I can proceed with the same on the lower. I saw him Monday and he is continuing to do well with the upper. I’ll keep you informed as Mr. A’s treatment continues.

I began another interesting case last week. As this is our second case report we will, with great originality, call this Mrs. B. Mrs. B is a 75 year old woman in good health who came to me with missing back teeth and a fixed bridge on the upper and lower that went from the eye tooth on one side to the eye tooth on the other; 6 unit bridges. Decay had gotten under all 4 crowns on the eye teeth and the bridges or teeth could not be saved.

To make a long story short, it turned out that Mrs. B wanted her remaining teeth removed and wished to have the All-on-Four procedure on the upper and lower, i.e. full sets of upper and lower non-removable teeth made of 4 upper and 4 lower implants. The problem here was that the front teeth that were replaced by the upper fixed bridge had been extracted many years ago and the bone in that area had dissolved, leaving a ridge of only 4 millimeters in width. Mrs. B’s previous dentist told her that there was not adequate bone volume for implants and that she would either need extensive bone grafting, using her hip as a donor site or just give up and wear dentures. These were not attractive alternatives to Mrs. B so she came to me for a consult.

We now have a couple of bone grafting materials that work just about as well as natural bone, possibly better, that require no secondary surgery to harvest material for a graft. But as it turned out, Mrs. B could be treated adequately without bone grafting of any nature, so I’ll talk about the new bone grafting materials in another blog. When teeth are removed, the bone remaining that once held the roots of the teeth is called a ridge. To place a large enough implant to secure a non-removable set of teeth, the ridge needs to be a least 7 millimeters wide. As I mentioned, Mrs. B‘s ridge was only 4 mm wide. Keep in mind that bone has a consistency more like wood than concrete, so it is possible in certain circumstances to “stretch” or expand it. In this case, by making a very thin incision in the center of the bone ridge and then placing a small tapered screw into the bone, I was able to expand it to 7mm and place the implants. This saved Mrs. B two additional surgeries, months of healing time and a lot of expense. Though the success rate for this procedure is very high, it is not without possible complications, so I’ll let you know how it goes.

All on Four dental implant case for a patient that experienced uncontrollable tooth decay due to head and neck radiation for oral cancer.

Good morning everyone. I promised a report this week on the case I mentioned last week. That was the patient requiring full mouth reconstruction with the All-on-Four technique because of rampant and uncontrollable tooth decay as a result of head and neck radiation therapy for oral cancer. We’ll call this gentleman Mr. A as he is the first of our case reports. As you may recall, I decided to do the upper only and proceed with the lowers later after appropriate hyperbaric therapy. Mr. A arrived at 8AM Monday morning to begin his sedation. At 9AM, the surgery had begun. Under conscious sedation, 8 upper teeth were extracted and 4 dental implants were placed. The surgery was completed at 11:15 AM and we began making a new set of teeth. Mr. A rested comfortably and at 4PM we let him see his new smile. Mr. A loved it and reported the following day that he was pain free and working in his garden. I expect a continued uneventful recovery. Regarding the need for hyperbaric oxygen treatment before I proceed with the lower, there is an interesting side note. This has often been a major inconvenience for my patients because of the expense – often in excess of $10,000.00. Mr. A found that Medicare will cover the cost. That’s a welcome development. As this case moves to completion, I’ll keep you up to date. Mr. A’s next visit will be in about two weeks. We’ll check on healing and give him oral hygiene instruction. I’ll let you know how he is. Visit www.nodentures.com to learn more about the All-on-4 procedure.

I encountered an interesting challenge a few days ago and did the follow up consultation yesterday. This patient was a healthy active 72 yr. old gentleman missing only a couple of teeth that had been replaced with fixed bridges and his remaining teeth were all crowned. Most of this work had been completed about 7 yrs ago and was beautifully conceived, planned and crafted. Unfortunately, there was recurrent tooth decay under almost every crown and several teeth were actually broken off.

The medical history revealed that this gentleman had been treated for cancer on the border of his tongue about 15 years ago with radiation and chemo. Dry mouth and other factors related to this treatment regimen lead to his rampant tooth decay. Neither he nor I had any confidence that if we removed and replaced all of his existing crowns and bridges that the result would be lasting. That combined with the need for surgery and dental implants, even if we did attempt a crown and bridge solution, led us to favor an approach that would ultimately involve removing all of his existing teeth and replacing them with implants and a non-removable set of teeth that would attach to them. This would require hyperbaric oxygen for the lower arch but not the upper.

We decided to do the upper arch this Monday with the classic All-on-Four procedure and work with the lower in a few weeks. Monday, I will extract all of his upper teeth, place four implants, and construct and deliver a full set of non-removable teeth. It will take most of the day, primarily to make the new teeth, but he will be sedated, so the day will go quickly and easily for him.

The primary risk for this patient is the possibility that I will not be able to attach his new teeth to the implants the same day and he may have to wear an upper denture for six months first. This risk is greatest when most of the upper teeth remain and require extraction on the day of implant placement. The tooth sockets leave little bone to engage the implants and sometimes, approximately 3% of the time, the implant cannot be placed sufficiently tight and I must wait before attaching anything to it. I’ll try to give you a report next week on how this goes.
